Resumen
Se propone investigar efectos de la inhibición del sistema renina-angiotensina -ISRA- sobre cognición -memoria de trabajo, de largo plazo- en pacientes con hipertensión esencial. Efectos preservadores de ISRA sobre los sistemas cardiovascular y renal han sido demostrados en ratas envejecidas. Datos iniciales muestran que una ISRA de largo plazo mejora la memoria de trabajo y de largo plazo espacial, y aminora el estrés (Basso, Altamirano, Lores Arnaiz, 2000). Se han hallado efectos nootrópicos en tratamiento agudo con ISRA (Raghavendra et alt, 1998). Pacientes hipertensos tratados con y sin ISRA (N=60) serán evaluados en personalidad, ansiedad, enojo y afrontamiento al estrés. Se evaluarán su memoria de trabajo y de largo plazo. Se medirá la reactividad hipertensiva -elevación de la presión sanguínea- a un estresor psicológico mediante instrumental psicofisiológico "Dr. Schuhfried" (Vienna Test), de acuerdo a la hipótesis de Diamond et alt (1996) de que el estrés deteriora la memoria de trabajo. Sujetos apareados por reactividad se asignarán aleatoriamente a tratamiento de reducción de estrés con biofeedback, o no tratamiento. Se repetirá la evaluación en memoria y reactividad al finalizar las diez semanas de tratamiento.. Se predice un efecto de interacción ISRA-reducción de estrés sobre la cognición.

Hypertension: psychological profile, stress and cognition in patients treated with and without inhibitors of the renin-angiotensin system

The aim of this research is to analyze the effect of long-term inhibition of the renin-angiotensin system -RASI- on both working and long-term memory in hypertensive patients. Previous research has found that RASI preserves cardiovascular and renal systems in aged rats (Basso, Kurnjek, Paglia, Müller, 1999). Recent data suggest that chronic treatment with RASI improves spatial working and reference memory, and reduces emotionality (Basso, Altamirano, Lores Arnaiz, 2000). RASI also has memory enhancing properties when it is acutely administered at a low dose (Raghavendra et alt, 1998). Personality, anger, anxiety and coping will be assessed in a sample of hypertensive patients treated with and whithout RASI (N=60). Patient´s working and reference memory will also be evaluated. Hypertensive stress reactivity will be measured with "Dr Schuhfried - Vienna Test physiorecorder", in accord with Diamond´s et alt hypothesis of stress impairment of working memory. Subjects will be matched by their physiological reactivity level and then, assigned to either a stress reduction group with biofeedback or to any treatment. It is predicted an interaction effect RASI by stress reduction treatment on patients cognition.
