TP63. Rol del Medico Pediatra y su Relación con los Especialistas. Especificidad de la Mirada Clínica.

Compararemos tres cohortes de médicos clínicos pediatras que ejercieron o ejercen su actividad en diferentes época, definidas en relación al grado de especialización de la Medicina y a la preocupación por la recuperación de una asistencia integral desde una Medicina General.

Compararemos la actividad desarrollada en las consultas e interconsultas por clínicos pediatras y por especialistas pediátricos.

Relevaremos a través de lo registrado en las Historias Clínicas las diferencias en los recorridos diagnósticos y terapéuticos según si la primera consulta fue asistida por un clínico o un especialista pediátrico.

Analizaremos la incidencia del gran desarrollo de las especialidades por un lado, y de la revalorización de la Medicina General por otro, en la modalidad asistencial.

Analizaremos también la influencia del modo de organización hospitalario en esa tarea.

Evaluaremos si afecta la asistencia el hecho que la primera consulta sea realizada por uno u otro de los profesionales mencionados.

Conceptualizaremos el sistema de creencias y valores que orientan la búsqueda diagnóstica y las terapéuticas del clínico pediatra.

Role of the paediatrician and his/her interaction with specialists in the everyday hospital practice. The specificity of the clinical approach.

Considering the strong development of medical specialties and subspecialties, and the decay of the profile of the general practitioner which took place during the last decades in our country, and with the purpose of evaluating their current role with regards to Hospital work and interactions with specialists, as study on the pediatricians behaviour and expectations during their practical work in Hospital will be carried out.

Information will be obtained from the current literature, personal interviews, observation of medical decision- making meetings, interactions of pediatricians with specialists and analysis of the medical records. Hospitals with different types of organization will be selected. The path followed by the patient according to the profile of the doctor in charge of the first appointment (pediatrician or specialist) will also be evaluated. The ways in which the medical care process develops in the everyday hospital practice, as well as the values regarding the performance of pediatricians will be defined and better delineated. The information obtained with the present project may also be useful for the analysis of the role of the general doctor in the adult medicine.
