Diferentes trabajos encuentran relación entre las alteraciones en la
teoría de la mente (TdM) y apraxia en pacientes con trastornos del espectro
autista (TEA). Chakrabarti et al., (2006) y Meltzoff y Decety (2003) proponen
que el sistema de neuronas en espejo (NE) es subyacente a la capacidad de Teoría
de la Mente (TdM). La TdM está deteriorada en la demencia frontotemporal
variante conductual (DFTvc). Varias investigaciones estudiaron la relación
existente entre la TdM y el reconocimiento de emociones en sujetos normales
(Bechara, 2002) y en sujetos con diferentes afecciones (Schroeter et al., 2008),
sin embargo no se dispone de investigaciones sobre estos procesos en pacientes
con DFTvc. Aunque la relación entre apraxia y demencia frontotemporal
(DFT) ha sido mencionada en el estudio de Pick (1906) y estudiada en variantes
temporales de las DFT (Josephs et al., 2007), la presencia de apraxia en pacientes
con DFTvc ha
sido escasamente reportada en la bibliografía. El objetivo de este trabajo
será estudiar si la relación entre alteraciones de la TdM, emociones
básicas y apraxia se encuentra también en los pacientes con DFTvc.
Y se postulará la existencia de un sustrato anatómico común
para dichas funciones siguiendo la teoría de simulación mental.
Se estudiaría en pacientes con DFTcv: a) la presencia de apraxia, b)
la relación entre apraxia y las alteraciones en TdM, c) la relación
entre las apraxias y las alteraciones del reconocimiento de emociones básicas,
d) la relación entre las alteraciones de TdM y del reconocimiento de
emociones básicas y e) la existencia de correlaciones anatómicas.
Se evaluará a los pacientes con exámenes neurológicos,
estudios de neuroimágenes y una batería neuropsicológica
de rutina para el diagnóstico de demencia. Luego se les administrará
una batería de praxias (Politis & Margulis, 1997; Politis, 2003),
Falsa creencia de primer orden (Wimmer & Perner, 1983); Faux Pas (Stone,
Baron-Cohen & Knight, 1998; Baron-Cohen et al., 1999) y Lectura de la Mente
en los Ojos (Baron-Cohen et al., 2001) y pruebas de reconocimiento de emociones
básicas en el rostro (Ekman & Friesen, 1976). Se espera que en los
pacientes con DFTvc, se halle apraxia con alteración en las pruebas de
comprensión e imitación gestual. Se cree que esto correlacionará
con TdM y con la alteración en el reconocimiento de emociones. Se espera
encontrar áreas deficitarias comunes en los pacientes con DFTvc que presentan
apraxia.

MENTAL SIMULATION IN PATIENTS WITH BEHAVIORAL VARIANT OF FRONTOTEMPORAL DEMENTIA
Different studies found a relationship between alterations in theory of mind
(ToM) in patients with apraxia and autismo spectrum disorders (ASD). Chakrabarti
et al, (2006) and Meltzoff & Decety (2003) propose that the mirror neuron
system (MNS) underlies to the ability of Theory of Mind (ToM). The ToM is impaired
in behavioral variant of frontotemporal dementia (bvFTD). Several studies examined
the relationship between ToM and emotion recognition in normal subjects (Bechara
, 2002) and in people with different disease (Schroeter et al, 2008), however
is no research on these processes in patients with bvFTD. Although the relationship
between apraxia and frontotemporal dementia has been mentioned in the study
of Pick (1906) and has been studied in temporal variations of DFT (Josephs et
al, 2007), the presence of apraxia in patients with bvFTD has been scarcely
reported in the literature. The aim of this work is to study whether the relationship
between disorders of ToM, basic emotions and apraxia are also found in patients
with bvFTD. Hence we postulate the existence of a common anatomical substrate
for such functions following the theory of mental simulation. It will be studied
in patients with bvFTD: a) the presence of apraxia, b) the relationship between
apraxia and alterations in ToM, c) the relationship between apraxia and disorder
of basic emotions recognition, d) the relationship between changes ToM and recognition
of basic emotions, e) the existence of anatomical correlations. Patients will
be assessed
with neurological examinations, neuroimaging and a neuropsychological battery
for routine diagnosis of dementia. Then they will be given a battery of praxias
(Politis & Margulis, 1997; Politis, 2003), false belief task (Wimmer &
Perner, 1983); Faux Pas (Stone, Baron- Cohen & Knight, 1998; Baron- Cohen
et al, 1999) and Reading the Mind in the Eyes (Baron - Cohen et al, 2001). Our
hope is to find apraxia with low performance on tests of comprehension and gestural
imitation. We believe that this will correlate with theory of mind and impaired
emotional recognition and that eventually we will find commons deficit areas
in patients with bvFTD with signs of apraxia.
